Provo City School District

Wasatch Elementary School

Wasatch Parents/Guardians: It is well documented that a child’s ability to see greatly impacts his or her ability to learn. Vision screening plays a vital role in the early identification of visual problems that may negatively affect a child’s academic success.

Wasatch will be holding its vision screening event on Tuesday, September 14th, 2021.

All grades will participate in the screening for distance vision. If your child wears glasses or contacts, please make sure they are wearing them on the day of the screening.

If your child does not pass the vision screening, they will be rescreened by the school nurse and a referral will be sent to you. If you would like to opt-out of vision screening, please return this form to your student’s teacher. Thank you!
