As registration for kindergarten and 7th grade begins, we’d like to remind you of the required immunizations.

To attend kindergarten, a student must have written proof of receiving the following immunizations:

  • 5 DTaP
  • 4 Polio
  • 2 Measles, Mumps, Rubella (MMR)
  • 3 Hepatitis B
  • 2 Hepatitis A
  • 2 Varicella (chickenpox)

Proof of immunity to disease(s) can be accepted in place of vaccination only if a document from a health care provider stating the student previously contracted the disease is presented to the school.

To attend the 7th grade, a student must have written proof of receiving the following immunizations:

  • 1 Tetanus/Diphtheria/Pertussis (Tdap) booster ***Must contain pertussis***
    • Tdap required regardless of interval since last dose tetanus/diphtheria containing vaccine
    • Kindergarten dose does not count as booster requirement for 7th grade
    • Td is not acceptable
  • 1 Meningococcal Vaccine
  • 2 Varicella (chickenpox) or documentation of the varicella disease. This must be confirmed with documentation from a licensed healthcare provider.

Exemptions

To claim a personal, religious or medical exemption, the legal guardian must complete an online educational module (free of charge) and provide a copy of the certificate to the school. Information and the online module can be found on The Health and Human Services Website.

If you do not have access to a computer, the online educational module may be completed at the Utah County Health Department or at the school.

For a medical exemption, a written notice from a licensed health care provider must be provided. The notice should state the physical condition of the student, and why that vaccine would endanger the student’s life or health. Additionally, a copy of the online exemption certificate needs to be provided.

If you have a financial need for low cost vaccines, please contact the health clerk or the school nurse.
